Appointment Information

Health and Safety During your Visit

  • Adults and children age 2 and older are required to bring and wear masks at all times when in our office.
  • Please bring the no more than two adults to the appointment (avoid bringing siblings unless unfeasible).
  • We now offer Covid testing in the office if your child is symptomatic. If your child is asymptomatic (has NO symptoms), please check out the Indiana State Department of Health Covid Testing web page
  • We will ask that you reschedule your child's wellness appointment if you or your child have a fever greater than 100.5, a cough, or are quarantining due to exposure to someone with Covid-19.  We can schedule an ill appointment if needed.
  • Exam rooms are cleaned with disinfectant between each patient.
  • Please avoid touching surfaces and wash/sanitize your hands frequently.

Canceling or Rescheduling

There are times when you need to cancel or reschedule an appointment.  For convenience, you can do this 24 hours a day, 7 days a week.

WEEKDAYS, AFTER HOURS OR WEEKENDS, Call 317-621-9000 and Press Option 1

  • ILL APPTS:  Please cancel at least 2 hours before your appointment.
  • CHECKUP APPTS:  Please cancel by 4:30 p.m. the day before your appointment. Monday appointments must be canceled by 3:30 p.m. on Friday.

Fees:  We do not charge for the first time an appointment is failed or canceled without adequate notice. The second occurrence is $50 and $100 for each additional appt. This is calculated per child, not per family.   

We recognize with busy lifestyles it can be challenging to remember appointments that were scheduled days, weeks, or months earlier.  For this reason, we confirm all appointments with a text message to your cell phone number before your scheduled appointment, 1 week, 1 day, and the same day.  We ask for a confirmation to be indicated. These reports and confirmation responses are kept on file in our office.
