Send a Non-Urgent Request or Form to be Completed
C. SEND A NON-URGENT REQUEST BELOW
STEP 1. Use the same Advice APP our Nurses do! Visit our online Symptom Checker for easy access to answers on common child illnesses, medication dosing charts, and behavior, eating, and wellness topics.
STEP 2. If the app does not answer your question, continue reading about the health concerns that require an appointment. If your request is NOT on the following list, complete the form below it.
If your child has any health concerns listed below, they must be seen in the office. DO NOT SEND A MESSAGE, AS WAITING FOR A RESPONSE CAN DELAY CARE.
Call us at 317-621-9000 on weekdays to schedule an appointment. If your child has any of these concerns on the weekend, please visit one of our walk-in clinics.
