Co-Payments and Non Covered Services

Co-Payments and/or any Non-Covered service amounts are due at the time services are provided regardless of who accompanies the child to the appointment. If we do not have a contract with your insurance company payment in full is required at the time services are provided.  There will be an additional $10.00 charge for any Co-Payment and/or Non-Covered service that is requested to be billed to the guarantor on the account .

Deductibles

If you have not met your deductible, we will collect 50% of the office visit charges at checkout.
