Add Your Child’s Photo to their Electronic Health Record

Here is how to email a picture of your child so that we can attach it to their electronic health record.

1. Click on this link and open a new e-mail window
2. In the subject line, type your child's name and date of birth.
3. Attach your child's photo (may be a scanned snapshot or a photo taken with a cellphone - head shots are best)
4. In case we have a question, include your name and contact information in the body of the email.
5. Click the send button!
